How do you organise your ROM library?

Mine's a mess. Folders by system, sure, but then I've got multiple regions, hacks, translations, and duplicates everywhere. Some people use frontends like LaunchBox or EmulationStation, others just have meticulous folder structures.

What's your system? Especially interested in how people handle romhacks and fan translations—do you keep them separate or mixed in?
